Job Summary Effectively provides day‑to‑day management for SSVF staff, drawing on experience in homelessness prevention, rapid re‑housing, and transitional supportive services. Facilitates client linkages from intake and assessment through to attainment or maintenance of permanent housing, while promoting accessibility, outreach, case management, advocacy and wrap‑around supports. Responsibilities Hold a caseload for the SSVF program. Provide day‑to‑day operational activities, including maintaining and tracking SSVF documents. Lead the SSVF program team. Back‑up support for SSVF case management teams, acting as an additional resource for Case Managers. Coordinate client outreach and recruitment efforts. Designate incoming cases for intake to the Case Management team. Develop inter‑agency relationships to enhance referral processes. Review client files, providing guidance and direction to case managers. Resolve client issues raised by Case Managers. Conduct weekly SSVF team meetings. Monitor case managers to ensure all available services are provided to each veteran household on a regular basis, evaluating service quality and effectiveness. Review and approve check requests. Serve as liaison with local landlords, addressing landlord/tenant issues; develop and administer a mediation program, providing assistance in client needs. Maintain good landlord/tenant relations. Facilitate outreach activities, including presentations to service providers and potential landlords, community fairs, one‑on‑one engagement, and education on available housing resources. Assist in developing planning, evaluation, and implementation procedures for the SSVF program. Maintain records and prepare forms, reports, and correspondence related to the federal grant‑funded SSVF program. Attend and participate in public meetings and presentations to explain the SSVF program. Facilitate and encourage information exchange with local, county, and regional agencies. Facilitate coordination of services with community partners, as assigned. Maintain and update knowledge through conferences, seminars, and in‑service trainings, as determined with departmental leadership. Plan and conduct training programs, as assigned. Analyze and evaluate data, prepare reports, and make specific recommendations regarding the SSVF program. Provide quarterly summaries and documentation reports required by the SSVF grant. Complete the monthly report cover sheet and repository upload, along with all other required reporting. Coordinate outreach efforts, including establishing outreach plans and schedules to visit the VA, shelters, and other congregating locations, and develop communications with the VA and key housing providers. Screen potential program participants for eligibility using approved forms and verify participant information and income status with applicable agencies. Conduct housing and comprehensive supportive services assessments, developing a comprehensive Family Services Plan. Coordinate services between the program and community partners for persons served and their families/supporters. Work with persons served, family members, housing agencies, and professionals to revise service plans and develop placement, stabilization, and follow‑up plans. Provide or arrange for housing support and wrap‑around services, connecting to all eligible mainstream resources for transition or maintenance of permanent housing. Travel to housing sites to support persons served, monitor progress, and address any health, behavioral, or life‑skills issues. Communicate effectively under confidentiality rules with the client service team to rapidly address and correct issues with services or supports—especially living skills, employment, education, family, and socialization. Ensure appropriate use of and documentation of any temporary financial assistance. Ensure participants receive services that support rapid re‑housing—addressing assessed needs, promoting healthy decision‑making, achieving financial independence, and fostering relationships in the community. Ensure participant reassessments are completed according to specific SSVF requirements. Participate in program and professional supervision, attend required staff and Project Management Team meetings and training, and contribute to team‑based collaborative planning. Complete all appropriate reporting in HMIS and other systems as required. Qualifications Bachelor's degree in counseling psychology, rehabilitative counseling, or a related human services field is required. Minimum of two years experience working with homeless individuals and families, including veterans, is preferred. Veteran or member of a veteran family is a plus. Other Requires driving a personal vehicle for company business and must meet agency MVR guidelines. Valid driver's license in the jurisdiction of residence. Vehicle liability coverage if driving personal vehicle; occasional transport of clients may be required. May be required to drive a van. Acceptable criminal background check and OIG exclusion check. Negative drug screening. CPR/First Aid certification must be acquired and maintained. Physical Requirements Ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary. Physical ability to travel to assigned locations and to stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and perform basic and light home maintenance activities and operate office equipment. Operating office equipment that requires continuous or repetitive hand/arm movements. Ability to remain seated for extended periods of time.
