Work Environment\n

This role offers a remote work setting with an emphasis on flexibility to meet the needs of clients. Our Social Worker will primarily engage with seniors and their families through virtual visits, but may also occasionally work on-site in schools or community facilities as needed. Regular business hours are Monday to Friday, 8:00 AM to 4:30 PM, although adaptability for evening or weekend hours may be required in response to client needs. On-call availability may also be necessary for urgent situations, ensuring continuous support for those we serve.

Key Responsibilities\n

• Conduct comprehensive assessments of the psychosocial needs of seniors, considering their emotional, mental, and social health.

• Collaborate closely with clients and their families to create individualized care plans that align with their specific goals, preferences, and circumstances.

• Provide ongoing counseling and emotional support for clients and their families, assisting them through various transitions and life challenges.

• Act as an advocate for client rights, ensuring access to necessary community resources and services, which may include transportation, healthcare, and social engagement opportunities.

• Work collaboratively with interdisciplinary teams that include nurses, caregivers, and other healthcare professionals to streamline and enhance the delivery of care.

• Regularly visit clients to monitor their well-being, assess living conditions, and proactively address any safety issues or concerns.

• Facilitate effective communication among family members involved in the client's care, helping to resolve conflicts and build supportive networks.

• Educate clients and their families about available resources, support groups, and programs that can enrich their quality of life and social involvement.

• Maintain meticulous and accurate documentation of assessments, care plans, progress notes, and all relevant client interactions.

• Commit to ongoing education on social work practices, regulations, and ethical standards to ensure compliance.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n

The salary range for this position is USD 60,000 – 100,000 annually, commensurate with experience and qualifications.
