Job Title: Registered Nurse (RN), Community Health NurseDepartment: Home CareLocation: Rochester Regional Health Home Care - 330 Monroe Avenue Rochester, NY 14607Hours Per Week: Per Diem - As NeededSchedule: Days/Evenings, Saturdays and SundaysSUMMARYAs a Community Health Nurse, you will focus on providing nursing interventions, patient education, and care management to a variety of clients in our community. This promotes good health and one on one patient care in the comfort of their own home.RESPONSIBILITIESPatient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of carePlanning. Develop and document individualized care plans customized for each patient’s unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progressCommunication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly; coordinates client services and/or referrals in an efficient and timely mannerDocumentation. Adhere to required department and system protocols, regulations (local, state, federal) and education requirementsREQUIRED QUALIFICATIONSDiploma or associate’s degree in nursingRegistered Nurse license in New York StateValid NYS Driver’s LicensePREFERRED QUALIFICATIONSBachelor's Degree in NursingCompassionate, independent, and patient focusedComprehensive nursing assessments and documentation, decisive judgment and professional behaviorTwo (2) years' experience in home care preferredBilingual preferredAt least one year of nursing experienceEDUCATION:AS: Nursing (Required)LICENSES / CERTIFICATIONS:BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - NYS Office of ProfessionsPHYSICAL REQUIREMENTS:Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. It may be modified in the future and eligible for additional pay components.
