Licensure/Certifications: Valid IL PT license, State Licensure, CPR/BLS (if applicable)

Experience: Prior pediatric or general home health experience preferred

Hourly Pay Rate: $63.65

What You'll Do

• Assess, plan, and implement individualized physical therapy programs for patients
• Evaluate patient progress and modify treatment plans as needed
• Assist patients across a broad age range, from pediatric to geriatric populations
• Communicate effectively with patients, families, and healthcare teams
• Document all patient care activities accurately and timely in electronic medical records
• Ensure safe transfer, positioning, and mobility support for patients
• Promote best practices and adhere to facility policies and procedures

What You Need

• Current IL Physical Therapist license or eligibility to obtain
• Prior experience in home health or outpatient physical therapy preferred
• Ability to work independently and communicate clearly

• Pediatric or geriatric care experience
• CPR/BLS certification
