- Trach
- Observe, assess, and provide nursing diagnoses for patients.
- Counsel patients and families, deliver health teaching, and focus on health maintenance and illness prevention.
- Maintain a comprehensive clinical record for each patient under care, ensuring accurate and timely documentation.
- Provide progress notes to the patient's physician or podiatrist when the patient's condition changes, deviates from the plan of care, or every 90 days (home nursing agency).
- Assign tasks to home health aides, prepare written instructions, and supervise their activities in the home setting.
- Direct and oversee the activities of LPNs to ensure adherence to the care plan and quality standards.
- Administer medications and treatments as prescribed by the patient's physician or podiatrist, following all safety and regulatory guidelines.
- Act as the coordinator of the health care team to maintain seamless care across the continuum, ensuring effective communication and linkages between all care providers.
- Active and unrestricted Registered Nurse (RN) license in the state of Illinois.
- Graduate of an accredited nursing program.
- Previous experience in home health care or a similar setting is preferred.
- Strong clinical assessment and documentation abilities.
- Excellent communication and coordination skills to work effectively with patients, families, and health care teams.
- Compassionate approach to patient care with a commitment to health maintenance and illness prevention.
