Pediatric Ambulatory Clinical Pharmacist

Job Responsibilities This role assumes responsibility for medication management of assigned patients, including medication reconciliation, monitoring, and patient education. The pharmacist evaluates medication therapy for appropriateness based on patient health status, institutional guidelines, and cost‑efficiency measures, and remains involved in the education of pharmacy and medical residents. The pharmacist also provides expert content for order sets, benefit design, formularies, and other pharmacy related documents. Job Duties Provides pharmacokinetic consultation, obtains and uses appropriate laboratory data to evaluate drug selection, dosing, and communicates findings. Evaluates patient response to drug therapy. Documents interventions and conducts outcome studies to demonstrate patient outcomes. Assists with the development of pathways, best practices, and guidelines for the assigned population. Educates staff, students, patients, and health care providers; submits work for publication biennially. Mentors residents, students, and new staff to meet departmental goals. Ensures continuity of patient care beyond the hospital visit, through all transitions of care. Volunteers for additional duties when staffing shortages occur. Adapts communication and service delivery to meet the needs of specific patient populations. Other related duties as required. Qualifications Education : Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred. Completion of a PGY1 Pharmacy Practice Residency or equivalent (3 years related specialty experience) required; PGY2 Residency preferred. Work Experience : None required; on‑the‑job training provided. Certifications : Current pharmacy license in the state of practice required. Knowledge, Skills and Abilities (KSAs) : Proficient with Windows‑style applications and keyboard for data entry; effective verbal and written communication; strong leadership and teaching skills; computer dexterity for patient information retrieval. Physical and Environmental Demands Medium Work: occasional lifting and frequent light lifting, standing for prolonged periods, travel throughout facilities, exposure to blood, body fluid, tissue and communicable diseases.
