Pharmacy Technician Medicine Reconciliation

Hourly Pay Range: $20.99 - $32.53 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights: Position: Pharmacy Technician Med Reconciliation Location: Highland Park Hospital Full Time: 40 hours/week Hours: Monday-Friday, 10 Hour Shifts, 4 Shifts each Week (11am - 9:30pm, 12pm - 10:30pm), Rotating Every 3rd weekend (12pm - 10:30pm) Required Travel: No What you will do: Assist in the process of collecting Home Medication History of patients being admitted to the hospital. This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information. This work will primarily be conducted in the Emergency Department but may also include inpatient floors.
