Work Environment

This position is primarily onsite, involving home visits to provide therapy directly in the patients' residences. Our team is well-equipped to offer a cohesive work environment, enabling focused patient care without the distractions often found in institutional settings. You will have the autonomy to develop therapy sessions tailored to individual patient needs while benefiting from a structured administrative support framework to optimize your time and efforts.

Key Responsibilities

• Conduct thorough assessments of patients' physical conditions and rehabilitation needs to develop personalized therapy plans.
• Deliver hands-on therapy sessions, guiding an average of 7 patients daily through specialized mobility, strength, and rehabilitation exercises.
• Educate patients and their families about treatment plans, progress, and exercises to ensure active participation in the rehabilitation process.
• Maintain accurate patient records and documentation to monitor progress and facilitate effective communication with the care team and other health professionals.
• Implement evidence-based practice methodologies and utilize appropriate physical therapy tools and modalities to enhance treatment effectiveness.

Patient Population / Client Focus

As a Physical Therapist at Moving & Grooving, you will work closely with a diverse patient population, including individuals recovering from surgeries, injuries, or chronic conditions that affect mobility and daily living activities. Your patients may vary from the elderly needing rehabilitation after orthopedic surgeries to younger individuals requiring post-injury therapy. Your role is crucial in restoring their independence and improving their functional outcomes.

Qualifications

• Licensed Physical Therapist in New York State.
• Bachelor's Degree in Physical Therapy; Master's or Doctorate preferred.
• A minimum of 2 years' clinical experience in a relevant setting, preferably in home care or outpatient rehabilitation.

Preferred Experience

• Experience with a variety of physical therapy modalities and tools (e.g., resistance bands, therapeutic ultrasound).
• Pediatrics or geriatric care specialization.
• Proficiency in using electronic health record (EHR) systems to document patient care and assessments.

Compensation and Benefits
