Work Environment

The position is based onsite at our state-of-the-art Rehabilitation Center, equipped with advanced therapeutic tools and technologies. You will work alongside a multidisciplinary team of healthcare professionals, including physicians, nurses, occupational therapists, and speech therapists, dedicated to delivering comprehensive care. The facility is designed to facilitate both individual and group therapy sessions, providing an encouraging atmosphere conducive to healing.

Key Responsibilities

• Conduct thorough evaluations and assessments of patients' physical conditions and functional limitations through comprehensive screening protocols and observational assessments.
• Develop and implement individualized, evidence-based treatment plans that promote recovery and enhance mobility, taking into consideration each patient's unique goals and motivations.
• Administer a variety of therapeutic interventions, including manual therapy, therapeutic exercises, modalities such as electrical stimulation, and gait training, to address pain and physical impairments.
• Monitor progress meticulously, ensuring accurate documentation, analyzing outcomes, and adjusting treatment plans dynamically based on patient response.
• Engage in collaborative discussions with interdisciplinary teams to optimize patient outcomes through coordinated care strategies.
• Educate patients and their families on home exercise programs and practical strategies for continued progress beyond clinical settings, fostering independence.
• Adhere to facility protocols and local and state regulations, ensuring the highest standard of practice and patient safety is maintained.

Patient Population / Client Focus

In this role, you will primarily work with a diverse population of residents recovering from surgery, injury, or debilitating conditions that require rehabilitation services. This includes elderly patients, those transitioning from hospital care, and individuals with chronic pain or mobility challenges. Your work will directly impact their ability to regain independence and improve their daily functioning.

Qualifications

• Education: A Doctor of Physical Therapy (DPT) or a Bachelor's/Master's degree from an accredited Physical Therapy program.
• Licensure: Must hold an active and unrestricted Physical Therapy license in the state of New York.
• Skills: Strong clinical evaluation skills, effective communication abilities, and proficient interpersonal skills are essential for this role.
• Approach: A compassionate and patient-centered mindset is crucial for building rapport with residents and delivering effective care.

Preferred Experience

• A minimum of 2 years of experience in a skilled nursing facility, rehabilitation center, or similar clinical setting is preferred.
• Experience with specialized populations, including geriatric patients or those with orthopedic or neurological conditions, is highly desirable.
• Proficiency with electronic medical record (EMR) systems and documentation practices specific to rehabilitation therapy.
