Barton PT and Rehab Clinic is seeking a dedicated and experienced Physical Therapist to join our team in Middletown, New York. This full-time position offers the unique opportunity to deliver personalized and comprehensive physical therapy services to patients within the comfort of their homes. Work Environment

The position is entirely onsite, allowing for direct interaction with patients in their homes. You will work primarily Monday to Friday, from 9 AM to 5 PM, ensuring a balanced work schedule that facilitates both professional and personal growth. Our clinic's approach prioritizes a one-on-one treatment model, meaning you will never be double-booked, allowing you to invest quality time with each patient, building strong professional relationships.

Key Responsibilities

• Conduct thorough assessments of patients' physical capabilities, identifying specific therapy goals based on individual needs.
• Develop and implement personalized treatment plans aimed at restoring mobility, alleviating pain, and preventing further physical decline.
• Provide hands-on therapy sessions, utilizing therapeutic exercises, balance training, and functional mobility interventions tailored to patient specifications.
• Educate patients and their families on safe mobility techniques and effective home exercise programs to support recovery outside of therapy sessions.
• Collaborate with a multidisciplinary team, including nurses and other healthcare professionals, to ensure coherent and comprehensive patient care.
• Document treatment plans, progress notes, and therapeutic outcomes accurately and in compliance with facility regulations and best practices.

Patient Population / Client Focus

Our Physical Therapist will primarily work with adults and seniors who are recovering from various conditions, including post-surgical rehabilitation, chronic pain management, and mobility challenges stemming from age-related decline or neurological conditions. You will have the opportunity to make a direct and positive difference in their lives, supporting them on their journey to recovery and independence.

Qualifications

• A graduate of an APTA accredited Physical Therapy program (B.S., M.S., D.P.T. or equivalent).
• Current Physical Therapy license in the State of New York is mandatory.
• Current Basic Life Support (BLS) Healthcare Provider verification.
• Knowledge of the Physical Therapy Practice Act for New York and a strong understanding of ethical guidelines from the American Physical Therapy Association.

Preferred Experience

• Minimum of 2 years of practical experience in physical therapy, with a preference for candidates who have worked in home care, skilled nursing, or outpatient settings.
• Previous involvement in developing comprehensive rehabilitation programs aimed at diverse patient populations.
• Experience collaborating effectively with interdisciplinary teams to ensure holistic patient care.

Compensation and Benefits

Our clinic is pleased to offer a competitive salary ranging from $100,000 to $120,000 depending on experience. In addition to the salary, we provide a comprehensive benefits package that includes:
