Work Environment

This position is primarily onsite and involves direct patient interactions in a home care environment. You will work alongside other healthcare professionals, including occupational therapists, nursing staff, and support personnel, to ensure a holistic approach to patient care. Key Responsibilities

• Evaluate and assess patient conditions through comprehensive physical examinations and medical histories.
• Develop individualized treatment plans that address each patient's unique needs, mobility issues, and rehabilitation goals.
• Implement and supervise therapeutic techniques, which may include manual therapy, exercise programs, and pain management strategies.
• Consult with patients' families to educate them on therapy techniques and home exercises to foster patient engagement and progress.
• Set measurable goals for improvement and conduct regular follow-ups to monitor patient progress and adjust treatment plans accordingly.
• Document patient evaluations, treatment plans, and progress notes in compliance with healthcare regulations and standards.
• Stay current with the latest research and best practices in physical therapy to continuously enhance patient outcomes.

Patient Population / Client Focus

As a Physical Therapist at Echo Rehabilitation Center, you will work with a diverse population of patients recovering from various physical conditions, including post-operative rehabilitation, orthopedic injuries, neurological disorders, and chronic pain management. You will play a crucial role in guiding patients through their recovery journeys, helping them regain functionality and return to their daily activities with increased independence.

Qualifications

• A valid New York State Physical Therapist License is required.
• A Bachelor's Degree in Physical Therapy or related field.
• Minimum of two years of clinical experience in a physical therapy setting, preferably within home care or skilled nursing environments.

Preferred Experience

• Advanced certifications such as a Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) will be preferred.

Compensation and Benefits
