Work Environment

In this role, you will work onsite, directly engaging with patients in their homes, providing tailored therapies that meet their individual needs. Key Responsibilities

• Provide quality physical therapy services to an average of 7 patients daily, focusing on individualized care based on specific patient conditions.
• Conduct thorough assessments to understand each patient's unique situation, mobility issues, and treatment needs.
• Design and implement effective treatment plans, utilizing modalities such as therapeutic exercises, functional training, gait training, and more.
• Communicate openly with patients and their families about treatment progress, managing expectations, and addressing any concerns.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal recovery outcomes.
• Complete timely and accurate documentation of patient evaluations, treatment plans, and progress reports using our state-of-the-art electronic health record systems.
• Participate in interdisciplinary meetings to collaborate with other healthcare providers and develop comprehensive care strategies.
• Engage in continued education and training to stay current with the latest therapeutic practices and methodologies in physical therapy.

Patient Population / Client Focus

Your patients will include a diverse range of individuals dealing with various physical challenges, including post-surgical recovery, chronic pain management, neurological conditions, and orthopedic injuries. You will have the opportunity to foster meaningful relationships while supporting them through their recovery journeys, emphasizing functional independence and improving their overall well-being.

Qualifications

• A current and valid New York State Physical Therapist License is mandatory.
• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• At least two years of relevant clinical experience, preferably in a home health setting, is essential.

Preferred Experience

• Advanced certifications or specialized training in areas such as geriatrics, orthopedics, or neurological rehabilitation will be an advantage.
• Experience with a variety of therapeutic techniques and equipment commonly used in home care settings.

Compensation and Benefits
