Work Environment

This position is onsite, specifically tailored for home care, which enables you to work directly with patients in their residences. Your daily routine will provide a dynamic work environment where you can actively engage with patients within their personal spaces, adapting treatments based on their unique home circumstances. The work schedule is Monday through Friday from 9:00 AM to 5:00 PM, allowing a balanced approach to your professional and personal life.

Key Responsibilities

• Conduct thorough assessments of new patients to evaluate their physical abilities, limitations, and specific rehabilitation needs.
• Create and implement individualized treatment plans that utilize a variety of therapeutic techniques such as manual therapy, patient education, and tailored exercise programs.
• Monitor patient progress consistently, adjusting treatment plans as necessary to ensure optimal recovery outcomes.
• Engage patients in therapeutic exercises and activities that promote strength, flexibility, and overall functional mobility.
• Educate patients and their families on rehabilitation processes and techniques to foster compliance and encourage independence.
• Document patient progress accurately and timely in healthcare software systems, ensuring continuity of care and compliance with regulatory standards.
• Collaborate with other healthcare professionals, including occupational therapists and clinical staff, to ensure comprehensive patient care and support.

Patient Population / Client Focus

The primary client base for this role includes elderly patients, post-operative individuals, and those recovering from serious injuries or illnesses. You will work closely with patients of varying health conditions, tailoring rehabilitation programs to meet their unique needs. Your effectiveness in delivering care directly impacts their overall well-being and recovery trajectory.

Qualifications

• A valid New York State Physical Therapist License is required.
• A Bachelor's Degree in Physical Therapy is mandatory; advanced degrees such as an MSPT or DPT are highly preferred.
• A minimum of two years of professional experience in a physical therapy setting, demonstrating the ability to deliver high-quality care.

Preferred Experience

• Demonstrated experience in home health care or skilled nursing facilities is highly desirable.
• Familiarity with patient assessment tools, treatment planning software, and documentation processes.
• Strong skills in collaborating with multidisciplinary teams to improve patient care.

Compensation and Benefits

The physical therapist position offers a competitive salary ranging between $100,000 and $120,000 annually, commensurate with experience.
