Work Environment

This role is primarily onsite, providing in-home care to a diverse patient population across New Windsor and surrounding areas. You will experience first-hand the differences that tailored therapy services make in the comfort of the patient's own space. Key Responsibilities

• Conduct thorough patient evaluations, utilizing assessment tools to inform effective, individualized treatment plans based on each patient's needs.
• Implement and monitor personalized exercise programs and manual therapy techniques aimed at improving functional mobility and enhancing patients' daily activities.
• Educate patients and their families on rehabilitation techniques, self-management strategies, and lifestyle adjustments to promote injury prevention and optimize recovery.
• Collaborate with interdisciplinary teams to ensure cohesive treatment approaches and continuity of care throughout the rehabilitation process.
• Document patient progress and adjust treatment plans as necessary, ensuring compliance with regulatory and organizational standards.
• Participate in ongoing professional development and team meetings to stay current with industry best practices and contribute to an enhanced care culture.

Patient Population / Client Focus

You will primarily work with individuals recovering from orthopedic surgeries, sports injuries, falls, and age-related mobility issues. Our patient demographic ranges from pediatric to geriatric populations, providing unique challenges and opportunities for personalized interventions.

Qualifications

• Must hold an active New York Physical Therapist license.
• A Bachelor's degree in Physical Therapy; further education in the form of a Master's or Doctorate in Physical Therapy is highly regarded.
• Strong communication skills to effectively educate and engage with patients and their families.

Preferred Experience

• A minimum of 2 years of experience in a clinical setting, preferably in outpatient or home care environments.
• Prior experience with manual therapy techniques and exercise prescription for various patient populations.
• Familiarity with electronic health record (EHR) systems to maintain documentation and measure patient progress efficiently.

Compensation and Benefits
