Work Environment

This role is based onsite and involves providing physical therapy services in patients' homes, allowing for a personalized therapy experience in a comfortable environment. The full-time work schedule runs from Monday to Friday, from 9:00 AM to 5:00 PM, offering a structured yet flexible work routine conducive to work-life balance. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical and functional abilities to devise tailored treatment strategies.
• Administer a range of therapeutic interventions, including but not limited to manual therapy, therapeutic exercise, and patient education to improve mobility and functionality.
• Monitor and document patient progress using evidence-based methodologies to ensure measurable outcomes are achieved.
• Collaborate closely with interdisciplinary teams to coordinate care efforts, ensuring holistic support for our patients.
• Guide patients through prescribed home exercise programs and educate them on techniques to prevent injuries and optimize recovery.
• Maintain accurate documentation compliant with hospital policy and health regulations to support quality and continuity of care.

Patient Population / Client Focus

Your role will primarily focus on adult and geriatric patients recovering from various conditions, including surgical rehabilitation, neurologic disorders, and age-related mobility challenges. By establishing a trusting relationship, you will encourage these patients to actively participate in their recovery process, emphasizing their tailored goals and preferences along the way.

Qualifications

• A valid New York State Physical Therapist License is required.
• A Master's degree in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) is necessary for this position.
• A minimum of two years of hands-on physical therapy experience, particularly in home care or outpatient settings, is preferred.

Preferred Experience

• Experience working with a diverse patient population, including those recovering from orthopedic surgeries and neurological conditions, is highly desirable.
• Proficiency in utilizing therapy equipment and modalities relevant to patient rehabilitation.

Compensation and Benefits
