• Salary: $82,500 - $97,800 Per Year, Depending on Experience

Responsibilities:

• Assess patient conditions and develop individualized care plans focusing on comfort and quality of life.
• Maintain detailed medical records and communicate patient status to relevant parties, ensuring seamless care coordination.
• Serve as the primary liaison among patients, their families, and healthcare providers, offering compassionate support and education.
• Educate patients and families on health management, palliative care options, and preventive measures to manage symptoms effectively.
• Coordinate with healthcare professionals to ensure continuous, patient-centered care and efficient case management.
• Conduct regular home visits to monitor patient progress and adjust care plans as necessary.

Requirements:

• License: Active RN license in Arizona with no restrictions.
• Experience: At least 1 year of in-home hospice experience is required.
• Skills: Proficient in case management software, exceptional organizational and communication skills.
• Traits: Empathetic, detail-oriented, and able to operate autonomously in field settings. Strong ability to manage emotional situations with sensitivity.
