Intake Assessment Representative- Weekends

Position Summary Shift: Thursday – Monday. This position is responsible for initiating and coordinating referrals, gathering necessary referral and payor information, inputting data into EMR, verifying patient information and initial eligibility with patient and referral source, and coordinating and scheduling date for HH admission. The role ensures information obtained at time of referral is accurate and complete. Responsibilities Coordinate and process incoming referrals, including assessing for needs from other HS departments. Evaluate referral information and contact patient and/or referral source to determine initial eligibility for HH services. Coordinate and ensure insurance information is obtained and accurately recorded into EMR. Coordinate and evaluate patient information for referrals within guidelines. Accurately document referral information utilizing critical thinking skills. Triage and coordinate with appropriate team members for review if incomplete. Serve as a liaison between referral sources, MD office, patients/families, department supervisors, managers, insurance team, Access RNs to ensure timely patient care. Coordinate and communicate with appropriate team members to ensure service standards are met. Oversee all fax communications: triage faxes, contact referral sources to ensure complete referrals and coordinate the electronic referral system. Coordinate accurate insurance information and problem‑solve insurance eligibility with insurance team. Utilize critical thinking skills to coordinate and assign appropriate start of care date under clinical guidelines utilizing clinical staff as appropriate. Coordinate with referral sources, including hospital, physician, facility customers and insurance case managers to obtain additional information, confirm referral receipt and communicate start of care date. Measure productivity and manage team assignments as needed. Prepare specific reports as directed for distribution within Home Health and other departments. Stay current on CMS regulations and maintain current knowledge of Home Services products and services, and other referral sources. Seek at all times to enhance Home Services Departments relationship with referral and reimbursement sources. Participate in Access Services and Insurance Staff meetings and education sessions. Perform other duties as assigned. Education and Experience Required – Coursework/Training in medical terminology or equivalent education/experience. Preferred – Bachelor’s Degree. Skills Mathematical Skills: Ability to add, subtract, multiply, and divide all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Communication: Strong written and verbal communication skills. Computer Skills: Excellent computer skills, including proficiency with Microsoft Office Suite software and other relevant software programs. English Proficiency: Ability to understand, read, write, and speak English. Work Independence: Ability to work independently in a high-volume environment. Critical Thinking: Ability to think critically and work with complex details. Time Management: Excellent time management skills, able to establish priorities and multitask in a fast‑paced work environment. Problem Solving: Ability to problem‑solve and work effectively in a team. Organizational Skills: Excellent organizational and coordination abilities. Physical Demands and Work Environment The demands of this role require the employee to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. The employee may be exposed to bodily fluids, infectious diseases, and other conditions typical to the healthcare field. Routine use of standard medical equipment and tools associated with clinical care is essential.
