Key Responsibilities\n

• Independently manage a designated patient caseload, conducting in-depth evaluations to assess the social, emotional, and financial factors affecting patients’ health outcomes.

• Formulate and implement comprehensive treatment plans aimed at alleviating psychosocial barriers, fostering patient agency, and ensuring that individuals maintain a sense of control over their healthcare journey.

• Provide consultation and therapeutic interventions, applying various modalities to address challenging psychosocial situations, including crisis intervention and resource facilitation.

• Coordinate and collaborate with case managers, healthcare professionals, and community resources to ensure all psychosocial and cultural needs are incorporated into care and discharge plans.

• Supervise and mentor junior social workers, interns, and volunteers, delivering weekly reviews and promoting best practices in accordance with departmental standards.

• Engage in active learning by pursuing ongoing education around social work practices and empowering team members with essential knowledge regarding psychosocial elements impacting patient care.

• Participate in interdisciplinary meetings and patient care conferences to provide insights and recommendations pertaining to your caseload.

• Be available for on-call duties on a rotational basis to provide immediate support during crises.

You will be serving a diverse patient population at our Community Health Clinic in Stockton, California. Qualifications\n

• A valid and unrestricted license as a Clinical Social Worker (LCSW) in the state of California.

• A Bachelor’s Degree in Social Work or a related field; a Master’s Degree in Social Work (MSW) is preferred.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and qualifications.
