Experience Structured Living (ESL) provides structured transitional sober living support for adults working toward greater independence, stability, and long-term recovery. We are seeking a compassionate, organized, and dependable Temporary Integrated Care Manager (ICM) to support our clients and residential program in Oceanside. This temporary assignment will run from November through February. The ideal candidate is comfortable working directly with adults in recovery, coordinating multiple aspects of client care, maintaining appropriate professional boundaries, and helping clients develop the skills and accountability necessary for successful independent living.

ABOUT THE POSITION

The Integrated Care Manager performs advanced indirect and selected direct client care duties in support of each client's living-skills and recovery goals under the direction of the Aftercare Program Supervisor. The ICM provides general oversight of the housing environment, identifies and reports environmental or operational needs, and works closely with program leadership and other members of the care team. This position also provides case management services, including assessing client needs, coordinating care, assisting clients with treatment and recovery goals, facilitating transportation to appointments and activities, and supporting medication management processes.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Responsibilities include, but are not limited to:

• Ensure clients are awake and prepared for scheduled morning activities by 7:30 a.m.
• Facilitate morning meetings, meditation, household chores, and other structured activities.
• Work individually with clients to support treatment, recovery, and independent living goals.
• Assist with behavioral concerns and coordinate appropriate clinical or psychiatric support when needed.
• Coordinate and facilitate client transportation to treatment, outside appointments, recovery meetings, and scheduled activities.
• Support medication management processes and communicate medication changes, concerns, and incident reports to the appropriate Medical Services staff and program leadership.
• Conduct meal-planning sessions and assist clients with maintaining household routines.
• Manage designated weekly household expenses, maintain receipts, and accurately account for remaining funds.
• Maintain and update the Client Task Manager spreadsheet daily.
• Update the Master Schedule Board and Client Information Station with schedule changes, weekly menus, chore assignments, and other relevant information.
• Implement the weekly UA testing schedule and confirm completion as required.
• Coordinate preparation of housing accommodations for incoming clients.
• Support and monitor client participation in required outside recovery meetings.
• Collect and review meeting attendance sheets, phase-up requests, pass requests, and other required client documentation.
• Coordinate daily client store runs and appropriately manage program cash and/or credit cards when assigned.
• Complete thorough shift pass-down communication with other ICM staff using the designated pass-down process.
• Maintain communication with the Aftercare Program Supervisor and Operations regarding client, housing, safety, or environmental concerns.
• Maintain accurate, timely, and professional documentation.
• Perform other duties as assigned.

CLIENT POPULATION

ESL serves adults age 18 and older who meet DSM-5 criteria for substance use disorders, with or without co-occurring psychiatric conditions, as well as individuals experiencing psychiatric conditions with or without substance use disorders.

Temporary Assignment

This is a temporary position anticipated to run from November through February.

Requirements:

 

MINIMUM JOB REQUIRMENTS:
Education, Training, and Experience Required:

• High school diploma or equivalent and one (1) year of experience in behavioral health care, preferred.
• Attainment of at least 18 years of age
• A Health Screening with negative TB Test Results ? Current CPR/ First Aid certification.
• Drivers License- Transportation of clients required, must have active drivers license, with a clean mvr
• Should be experienced house manager or Mental Health Technician, preferably in the field of chemical dependency and/or behavioral health.

Required Knowledge, Skills, and Abilities:

• Knowledge of industry standards as related to the position of The Integrated Care Manager.
• Basic knowledge of PC software programs, particularly Microsoft Word and Excel is preferred.
• Written and oral communication skills; must be able to concentrate on work amid distractions such as noise, conversations and foot traffic
• Must maintain self-control in volatile or hostile situations such as when verbally or physically confronted in an aggressive manner
• Must be able to work independently with little supervision in an unstructured environment with multiple demands
• Must possess a reading and writing level that provides for clear expression and/or understanding of assignments related to job description.
• Ability to represent the program well in interactions both inside and outside the facility
