You will serve as the clinical bridge between referral and care delivery, completing comprehensive start-of-care assessments, establishing individualized care plans, and ensuring patients begin their home health journey with confidence and support. \n \n If you thrive in an autonomous role, enjoy building meaningful patient relationships, and are passionate about delivering high-quality care in the home setting, we invite you to join our growing team. \n Why Choose Elara Caring? \n \n \n Competitive compensation package (pay is based off experience with a range between $88K-$105K) \n \n Full benefits, including medical, dental, and vision coverage \n \n 401(k) with employer match \n \n Generous paid time off and paid holidays \n \n Tuition reimbursement and continuing education opportunities \n \n Career advancement and leadership development pathways \n \n Pet insurance and family/pet bereavement benefits \n \n Supportive leadership and collaborative clinical team environment \n \n The opportunity to directly impact patient outcomes in their homes \n \n What You'll Do \n \n \n Complete Start of Care (SOC) visits and comprehensive nursing assessments for new home health patients \n \n Evaluate patients' clinical, functional, psychosocial, and environmental needs \n \n Develop individualized plans of care in collaboration with physicians and interdisciplinary team members \n \n Accurately document assessments and admission information in the electronic medical record \n \n Provide patient and caregiver education regarding diagnoses, medications, treatments, and home safety \n \n Identify patient risks and coordinate appropriate interventions to support successful outcomes \n \n Communicate significant findings and changes in patient condition to physicians and clinical leadership promptly \n \n Ensure compliance with agency policies, state and federal regulations, and quality standards \n \n Support continuity of care through effective collaboration with case managers and branch clinical teams \n \n Maintain patient confidentiality and uphold HIPAA regulations at all times \n Qualifications \n \n \n Current, unrestricted Massachusetts Registered Nurse (RN) license \n \n Minimum of one (1) year of nursing experience required \n \n Previous Home Health experience required \n \n Prior admissions or Start of Care experience preferred \n \n Experience with HomeCare HomeBase (HCHB) preferred \n \n Associate Degree in Nursing required; BSN preferred \n \n Strong assessment, clinical decision-making, and documentation skills \n \n Ability to work independently while managing a dynamic daily schedule \n \n Valid driver's license, reliable transportation, and current auto insurance \n \n Willingness to travel throughout the assigned coverage area \n \n Join a Growing Team \n This position has been added to support the continued growth of our Massachusetts Home Health program.
