Nurse Practitioner

We are seeking a full-time Nurse Practitioner (40 hours per week) to join our Family Medicine practice at 21 Elm Street in South Deerfield, MA. Our hours of operation are Monday-Friday, 8:00am-5:00pm, with a weekly schedule of 36 hours of direct patient care and 4 hours of administrative time, designated on Fridays. Our South Deerfield office is a close-knit, community-centered practice dedicated to comprehensive primary care for patients of all ages. We provide physical exams, routine screenings, preventive care, and the diagnosis and management of acute and chronic conditions-with a strong focus on prevention and long-term wellness. Primary care is about relationships, and we take pride in truly knowing our patients to deliver exceptional, personalized care. This role will support a patient panel of approximately 1,000 patients. The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations. Does this position require patient care? Yes.

Essential functions include:

• Providing direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings.
• Performing complete histories and physical examinations.
• Ordering, interpreting, and evaluating appropriate laboratory and diagnostic tests.
• Developing appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
• Consistently providing high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries.
• Performing bedside procedures as are appropriate to the patient population.
• Ordering medications and writing prescriptions in accordance with organizational and regulatory policies and procedures.

Qualifications:

Education: Master's Degree Nursing required. Can this role accept experience in lieu of a degree? No.

Licenses and Credentials: Advanced Practice Registered Nurse [APRN-NP - State License] - Generic - HR Only required.

Experience: Experience as a Nurse Practitioner 0-1 year preferred. 2+ years of experience as a nurse practitioner in primary care preferred.
