Responsibilities Conducting initial assessments for clients transitioning off the CLASS waiting list Performing post-hospital assessments to ensure continuity of care Managing and completing annual renewal assessments Collaborating with a compassionate team to ensure the highest quality of care Qualifications Current RN license in the State of Texas Minimum 1 year of home care nursing experience Strong typing skills (35 WPM with 80% accuracy) and Excel proficiency A reliable vehicle, valid auto insurance, and willingness to travel throughout Austin A heart for service and a commitment to empowering others Why Join DSSW?
