Physician Job Requirements

Graduate of an accredited MD or DO program
At least 5 years of physician or clinical experience
Valid Indiana physician license
Board certification in Family Medicine, Internal Medicine, or Geriatric Medicine
Internal Medicine experience preferred
Ability to complete required tuberculosis screening for this patient-facing role
Strong commitment to patient-centered care and continuous improvement of the patient experience
Senior Primary Care Physician Responsibilities

Provide comprehensive outpatient primary care for senior and geriatric patients
Conduct wellness visits, physical examinations, health screenings, and preventive care evaluations
Diagnose and manage acute illnesses, chronic conditions, and minor injuries
Develop individualized treatment plans and coordinate care with specialists when appropriate
Maintain accurate medical histories and timely clinical documentation
Support continuity of care through strong, lasting patient relationships
Address the physical, emotional, social, and behavioral factors that influence patient health
