Job Description
Title: Physical Therapist - Home Health

Schedule:
- 8-hour day shifts only
- Weekends: Not required

Assignment Details:
- Patient population: Adults / Geriatrics
- Typical diagnoses: dementia, Parkinson's, ALS, MS, ortho post-op, post-stroke
- Visits: May see up to 6 patients per day; productivity points standard

Requirements:
- Minimum 1 year of skilled home health experience
- Required certifications: BCLS / BLS
- Preferred certifications/skills: Wound care, PTA credential, OASIS documentation, billing/home health experience
- EMR: EPIC Remote Client for Home Health and Rover

Roles & Responsibilities:
- Conduct home health evaluations and complete OASIS documentation.
- Implement therapy plans, including transfers, gait training, and home exercise programs.
- Provide basic wound care and monitor skin integrity.
- Assist patients with neuro (stroke, Parkinson's, ALS, MS) and orthopedic rehabilitation.
- Schedule appointments and order necessary supplies.

Compliance / Notes:
- Confirm vaccine requirements with hiring manager.
- Clinician expected to provide own transportation; occasional long drive days.
- Confirm floating expectations due to conflicting statements.
