As a Pharmacy Technician – Care Manager, you will work closely with healthcare teams and patients to optimize outcomes and improve overall health. Responsibilities include conducting medication reconciliation and resolving discrepancies with care teams, educating patients on medication use, side effects, and adherence strategies, monitoring medication adherence and follow up with at-risk patients, supporting care transitions, including refills, prior authorizations, and delivery coordination, maintaining accurate documentation in EHR systems, collaborating with pharmacists to identify drug interactions and improve care plans, ensuring compliance with HIPAA and pharmacy regulations, and participating in quality improvement initiatives.

Requirements include certification with a current Pharmacy Technician License from a recognized certifying body, previous experience in a pharmacy setting, ideally with a focus on care management or chronic disease management, strong understanding of pharmacology, medication therapy management, and patient-centered care practices, technology proficiency with EHR systems, pharmacy software, and telehealth tools, excellent verbal and written communication skills for patient and provider interactions, exceptional accuracy in documenting patient information and managing medication orders, and a proactive approach to identifying and resolving medication-related issues.
