Description

• Conduct thorough evaluations in inpatient and outpatient settings to create personalized treatment plans
• Implement targeted physical therapy interventions aimed at enhancing mobility, strength, balance, and functionality
• Work collaboratively with nurses, physicians, case managers, and other therapy professionals to achieve optimal patient outcomes
• Educate patients and their families about therapeutic goals, discharge plans, and prescribed exercises to continue at home
• Keep meticulous records of evaluations, treatments, and progress in line with facility standards and regulatory requirements
• Engage in care team discussions and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from a recognized institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., in geriatrics or neurology) are welcome but not mandatory

Skills

• Prior experience is highly desirable
