Role Description

• Assess patients in both inpatient and outpatient environments while crafting personalized treatment blueprints
• Implement physical therapy techniques aimed at enhancing mobility, building strength, improving balance, and promoting overall function
• Team up with nurses, physicians, case managers, and other rehabilitation experts to enhance patient outcomes
• Guide patients and their families regarding therapeutic goals, discharge planning, and at-home exercises
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in following facility as well as regulatory protocols
• Engage in care team rounds and interdisciplinary conferences to ensure cohesive care delivery

Qualifications

• Doctorate in Physical Therapy (DPT) or another equivalent degree from an accredited institution

Certification Requirements

• Current state licensure as a Physical Therapist
• CPR certification preferred
• Specialized credentials (such as neurological, geriatric) are advantageous but not necessary

Essential Skills

• Prior experience is strongly preferred
