Roles and Responsibilities
- Conduct thorough evaluations of patients in a range of settings, crafting personalized treatment plans that cater to their specific needs
- Deliver expert physical therapy interventions designed to enhance mobility, strength, balance, and overall functionality
- Work seamlessly with fellow healthcare professionals, including nurses, doctors, case managers, and rehabilitation experts to maximize patient outcomes
- Educate patients and their families about therapy goals, discharge plans, and at-home exercise routines
- Ensure precise documentation of evaluations, treatments, and progress in alignment with facility protocols and regulatory standards
- Engage in collaborative care team rounds and interdisciplinary meetings to assure smooth coordination of care
- Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution
- State licensure as a Physical Therapist
- CPR certification preferred
- Additional specialized certifications (e.g., neurological, geriatric) are welcome but not essential
- Prior experience in a similar role is highly desirable
- Strong competency in managing patients with complex medical issues and mobility challenges
- Excellent communication and collaboration capabilities
