Moms In Motion - A Virginia Service Facilitation Provider
Moms In Motion is seeking a Service Facilitator (SF) Case Manager to support Virginia families navigating CCC+, CL & FIS, EPSDT waiver programs. You’ll travel locally to client homes and work remotely from a laptop, helping families access services and create meaningful care plans.
