Key Responsibilities\n

• Serve as a liaison between clients, their families, and external outpatient providers, ensuring seamless communication and coordination of care.

• Engage with insurance companies to manage pre-certification processes and utilization management, streamlining access to necessary services for clients.

• Participate actively in discharge and aftercare planning, providing clients with the resources they need for continued success post-treatment.

• Offer therapeutic meal support by modeling healthy relationships with food and encouraging positive behavioral patterns.

• Provide empathetic, safe, and affirming care tailored to the unique experiences and intersecting identities of each client.

Qualifications\n
