CHHA Account Manager

Your Choice At Home is seeking an Account Manager to help grow our home health services by building strong relationships with healthcare professionals, referral sources, community organizations, and prospective patients.

This position focuses on relationship management, business development, community outreach, and generating new patient referrals. This is not a Home Health Aide recruitment position.

Productivity Bonus

• Eligible for a performance-based bonus tied to productivity and successful patient growth.

Key Responsibilities

• Develop and maintain relationships with hospitals, rehabilitation facilities, physicians' offices, social workers, discharge planners, assisted living communities, and other referral sources.
• Identify and develop new referral opportunities to increase patient census.
• Conduct regular phone, email, and in-person outreach with referral partners.
• Educate referral sources, patients, and families about Your Choice At Home services.
• Follow up on leads and referrals and help connect prospective patients with the appropriate team.
• Represent Your Choice At Home at community events, healthcare events, networking opportunities, and other outreach activities.
• Maintain strong relationships with existing accounts and identify opportunities for continued growth.
• Track referral activity, account relationships, and outreach efforts.
• Assist with promotional materials, social media, and other outreach initiatives as needed.
• Collaborate with leadership and internal teams to support growth and ensure a positive experience for referral partners and patients.

Qualifications

• Experience in healthcare, home health, sales, business development, account management, or community outreach preferred.
• Strong communication, networking, and relationship-building skills.
• Comfortable making phone calls and conducting in-person visits.
• Self-motivated, organized, and able to manage multiple relationships and priorities.
• Professional, outgoing, and comfortable representing an organization in the community.
• Healthcare or home health experience is a plus.
• Bilingual or multilingual candidates are encouraged to apply.
• Valid driver's license and reliable transportation may be required.
