Salary Hourly rate: $35.64 - $49.89. Placement within the salary range depends on factors such as relevant work experience and internal equity. For union positions, placement follows the collective bargaining agreement. Benefits

Matching 401(k) Excellent medical, dental and vision coverage Life insurance Annual Employee Salary Increase and Incentive Bonus Paid time off and Holiday pay Description A licensed, masters-prepared social worker who is an experienced independent generalist clinician responsible for complex psychosocial interventions and facilitating discharge planning and resource utilization for all patient populations throughout the Northwestern Memorial Hospital campus. Proactively collaborates with Patient Care staff, Medical Staff, and other ancillary departments, integrating the Department’s functions with other aspects of patient care. For each problem, issue, or concern, develops observable and measurable goals and expected outcomes in areas of problem resolution, utilization and resource management, and patient satisfaction. Responsibilities Conduct comprehensive psychosocial assessments with patients/families, including social, emotional, cultural, mental status, environmental and financial circumstances. Recommend a plan of intervention based on mutually established goals. Provide psychosocial interventions such as: Reactions to illness and disability, especially for the chronically and terminally ill. Facilitation of informed decision making (including advanced directives) and development of treatment/intervention plans. Adjustment to the hospital setting and compliance with treatment plan. Adjustment/coping with post‑hospital care needs and linkage to community resources. Gynecological/obstetrical-related issues (e.g., teen pregnancy, parenting issues, adoption planning, infant developmental problems, drug‑exposed neonate, fetal death, unplanned pregnancy, pregnancy termination). Insurance coverage and payment issues. Psychiatric symptoms and chemical dependency. Conflict resolution. Family and personal relationships impacting the plan of care & discharge plans. Assess physical environment and adequacy of support systems for outpatients to prevent crisis and/or hospitalization. Provide crisis intervention and/or protective services for: The elderly without support systems, with impaired mental status and/or victims of suspected abuse/neglect. Victims of suspected sexual/physical assault (includes rape and molestation). Victims of suspected child abuse or neglect. Victims of domestic violence. Guardianship and/or protective services for patients with significant mental status impairment or unsafe living environment. The homeless. Actively participate in discharge planning, ensuring coordinated communication to physicians, healthcare team, patients and families. Initial screening of all new patients for medical necessity, source of funding, and need for social work/planning services. Serve as point person for discharge planning, facilitating continuous communication and decision making, including multidisciplinary rounds and case conferences. Coordinate action plans when barriers are present to facilitate resolution. Coordinate discharge planning to ensure timely discharge and post‑hospital care needs assessment. Collaborate and communicate with multidisciplinary team at all phases of discharge planning. Proactively identify and resolve delays and obstacles to discharge. Consult and refer to appropriate disciplines/departments as required to expedite discharge. Demonstrate knowledge of community resources and connect patients/families with them. Act as advocate for patients requiring assistance to access needed resources or services. Facilitate review of high‑risk cases by Office of General Counsel, Risk Management; inform appropriate members of the healthcare team. Provide patient and family education promoting wellness and increasing knowledge of healthcare system. Complete documentation of activities in medical record and hospital‑wide information systems. Demonstrate knowledge of the utilization management process (level of care assignment, communication with payors, benefit authorization). Assist in collection and reporting of financial indicators (LOS, avoidable days, resource utilization, discharge barriers, cost per case, readmission rates, denial and appeals). Use data to drive decisions and implement performance improvement strategies related to case management for assigned patients/units. Qualifications Licensure in Illinois: Licensed Clinical Social Worker (LCSW). Master’s Degree in Social Work from a CSWE‑accredited school. Minimum two years post‑graduate experience in hospital social work or related settings. High level of interpersonal skills to affect positive outcomes. Organizational skills to prioritize and manage an appropriate caseload. Analytical skills to collect, analyze, interpret data, resolve problems, negotiate in sensitive situations. Self‑direction in daily work. Compliance with Northwestern Memorial Hospital policies on patient confidentiality, HIPAA, and professional conduct.
