CLINICAL SERVICE DIRECTOR

Job Purpose The Clinical Service Director (CSD) supervised by the VP of Clinical Services oversees day‑to‑day operations of the Interdisciplinary Team (IDT) and participant outcomes, coordinating integrated care in community settings, Day Health Center (DHC), Diagnostic & Treatment Center (DTC), and across the continuum of care. Job Responsibilities Take initiative, be resourceful, proactive, creative, innovative, and independent. Oversee, coordinate and support site‑based managers, clinical staff across DTC, DHC, rehabilitation, nutrition, social work, and home care services. Provide oversight of participants’ services and ensure development of personalized care plans with positive outcomes. Coordinate in‑service training and continuing education for professional development and advancement of team members. Collect data for operational metrics to ensure participants are treated at the appropriate level of care. Participate in quality assurance and regulatory initiatives to ensure compliance with agencies. Serve as administrator on call during non‑operational hours. Collaborate with departments to establish new and improved care delivery processes and treatment protocols in evidence‑based practice. Monitor and minimize voluntary disenrollment. Ensure all clinical documentation is complete, accurate and compliant with policies. Investigate incidents and resolve customer issues and grievances in a timely manner. Conduct random chart reviews for compliance and address deficiencies. Participate in special projects and assignments as required. Managerial Standards Ensure direct reports comply with rules, regulations, and policies. Evaluate work performance and competency; provide ongoing feedback and conduct annual performance reviews. Maintain accountability for staff performance and professional conduct. Apply disciplinary action policies and collaborate with Human Resources for guidance. Develop individual professional development plans and identify in‑service training needs. Investigate and resolve employee grievances with HR assistance. Participate in annual budgeting planning, oversee budget management, and approve departmental expenditures. Schedule 8:30AM – 5:30PM, Weekly Hours: 40. Qualifications Education: Bachelor’s Degree in Nursing (BSN) from a CCNE/NLN‑accredited school (required); BSN preferred. Completed NYSED‑approved infection control coursework. Experience: Minimum 3 years recent nursing experience in an acute care setting. Minimum 1 year clinical experience with the elderly population in a Certified Home Health Agency, Managed Long Term Care, or PACE. Minimum 2–3 years of clinical staff supervisory experience preferred. License: Current active and unrestricted RN license in NYS. Language: Bilingual preferred. Additional Requirements Be medically cleared for communicable diseases and keep all immunizations up to date before engaging in direct participant contact. Physical Requirements Stand for up to 6 hours, sit for 6‑8 hours, and lift/push/pull up to 50pounds. Move stairs, climb, and walk to work areas; travel between sites necessary. Execute fine motor tasks such as typing and operating medical equipment. Maintain adequate visual and auditory skills for accurate assessment, documentation, and communication. Demonstrate good decision‑making and rational processing for job functions. Salary Min–Max: $135,000.00 – $145,000.00.
