HERO Case Manager

$19 - $23 per hour

StephenCenter has served homeless and low-income individuals in Omaha since1984 with the mission to partner with the community, families and individual to overcome homelessness, addiction and poverty. ThePettigrew Emergency Shelter and Permanent Supportive Housing offeringsprovide safe and secure housing while individuals and families completean assessment of permanent housing options. All Stephen Center residentshave access to dual diagnosis support services via the HEROaddiction/recovery program. 100% of Stephen Center shelter supportivehousing participants are living in poverty. Housing transition supportidentifies poverty barriers that may impact one’s ability to achieve ormaintain permanent housing through case management, life skills, andbehavioral health services. Stephen Center serves approximately 250people daily in all programs. Purpose The HERO CaseManager plays a critical role in supporting a coordinated, client-centeredrecovery environment by integrating clinical, operational, andcommunity-based services. Working collaboratively across interdisciplinaryteams, this position ensures safety, accountability, and continuity of carefor individuals navigating co-occurring substance use and mental healthrecovery. The role focuses on connecting clients to appropriate referrals andresources that support both immediate needs and long-term recovery, whilepartnering with clinical staff, leadership, and administrative teams tomaintain a safe and structured program environment. Through ongoingassessment, the Case Manager identifies client needs related to physical andmental health, housing, employment, family support, and other essentialservices, and works closely with the HERO care team and community partners toimplement individualized solutions. This position supports clients inpreparing for independent living while ensuring all services, clientengagement, and outcomes are accurately and timely documented in alignmentwith program standards, compliance requirements, and overall quality of care. Key Accountabilities Maintain a safe, structured, andtherapeutic environment by consistently following all safety, security, andprogram guidelines, including class participation, behavior compliance, andclient activity as needed. Facilitate and coordinate dailyrecovery programming, including groups, meetings, and structured activitiesaligned with the HERO continuum of care (STR, IOP, OP). Complete comprehensive intakes andclient orientations, ensuring everyone is welcomed into the program withclear expectations, resources, and support. Develop individualized service planswithin 24 hours of admission, in collaboration with clinical staff andleadership, ensuring plans are person-centered, goal-oriented, and responsiveto client needs. Implement and manage service plansthrough ongoing engagement with clients, interdisciplinary teams, andcommunity providers; documenting services, progress, barriers, and outcomesin a compliant manner. Ensure all documentation is enteredinto the Electronic Health Record within 72 hours of service delivery,maintaining accuracy, compliance, and alignment with agency and regulatorystandards. Coordinate access to medical, dental,psychiatric, and community-based services, strengthening partnerships tosupport whole-person care and successful transitions. Support high-acuity anddual-diagnosis clients’ coordination of care to reduce attrition and enhanceretention. Assist clients in navigating systemsand completing applications related to healthcare, income, housing, and othersupports necessary for successful community reintegration. Participate actively ininterdisciplinary team meetings, contributing to case consultations, careplanning, and shared problem-solving to ensure continuity and quality ofcare. Support program operations throughdaily responsibilities including room checks, chore verification, medicationobservation, and reinforcement of program expectations and clientaccountability. Demonstrate strong communication,teamwork, and interpersonal skills, contributing to a positive, professional,and solution-focused team culture. Demonstratesstrong organizational and time management skills to effectively managemultiple responsibilities in a fast-paced environment, balancing immediateclient needs with long-term goals and outcomes. Uphold strict confidentiality andethical standards in all aspects of client care and program operations. Providesupport to program leadership as needed and contribute to ongoing programdevelopment excellence. Ensure all safety and security guidelines arefollowed. Coordination of all Recovery activities andmeetings. Proficiency with Microsoft Office and excellentwritten, oral and interpersonal skills. Other duties as assigned. Position Requirements Bachelor’s degree in social work, humanservices, psychology, or a related field; and/or equivalent experience. Experience working with individualsexperiencing homelessness, addiction, or mental health challenges preferred. Standing -Periodically Walking -Periodically Kneeling–Periodically Pushing/Pulling–Periodically Lifting upto 20 pounds -Periodically Conducts self in a manner of professionalism that is conducive to recovery from mentalillness, co-occurring disorders and overall wellness. Abides by all specific program and Agency procedures, policies, and requirements. An authentic andhonest employee plays a key role in the client’s recovery. Often, an employeewill inadvertently become a role model for the client, regardless of whetheran employee has direct contact with clients on a daily basis. Therefore, Stephen Center employees willconduct themselves with honesty and integrity.
