Key Responsibilities\n

• Conduct initial assessments and develop comprehensive treatment plans tailored to individual client needs, addressing areas such as emotional distress, social challenges, and behavioral health.

• Provide crisis intervention services through phone consultations and in-person appointments, ensuring clients receive timely support during emergencies.

• Utilize evidence-based methodologies when implementing therapeutic practices, including cognitive-behavioral therapy (CBT) and trauma-informed care.

• Regularly monitor and evaluate client progress, adjusting treatment plans as needed to ensure effectiveness in achieving personalized goals.

• Maintain meticulous records of client assessments, treatment plans, and therapeutic sessions in compliance with state and federal regulations.

• Participate in interdisciplinary case conferences discussing clients’ needs and progress, collaborating with colleagues to provide a well-rounded approach to care.

• Act as a liaison between clients and community resources, facilitating referrals to relevant services such as housing assistance, financial support, or additional mental health resources.
