Clinical Pharmacist — Home Infusion Expert
Option Care
Option Care Health is seeking a pharmacist to determine patient suitability for home care and to design and monitor pharmacy care plans. You will provide medication education to staff, patients, and caregivers, ensuring continuity of pharmaceutical care across home and other settings. The role requires a PharmD, active license, and experience in sterile IV admixtures or related areas. ASHP residency or APPE or equivalent is preferred, with at least 1 year in related fields. #J-18808-Ljbffr Option Care
