Watch our “Now Hiring” TV commercial: Responsibilities: * Delivering skilled, compassionate in-home physical therapy services. * Conducting comprehensive initial evaluations, ongoing reassessments, and detailed fall risk assessments. * Designing and executing individualized treatment plans focused on balance, strength, and spatial safety. * Empowering patients and their multi-generational caregivers through tailored home exercise program education. * Ensuring accurate, compliant clinical documentation and collaborating smoothly with our interdisciplinary care team. What We Are Looking For: * Active, Hawaii license as a Physical Therapist (PT). * Minimum of one year of clinical experience (home health experience is a huge plus, but we welcome strong clinical backgrounds). * Excellent communication, time management, and independent clinical documentation skills. * Valid driver’s license and reliable transportation to travel to patient homes. * A genuine dedication to serving seniors and giving back to our local Oʻahu community.
