Facility Location

Schedule

• Shift: Night 5x8-Hour (22:45 - 07:15)
• Start Date: Every two weeks due to orientation schedule
• Weekend Requirement: Every other weekend as needed
• Consistency in attendance is essential. All shifts begin 15 minutes before and end 15 minutes after the scheduled time.

Job Details

• Patient Ratio: 1:18-20
• Type of Facility: State-Owned Long-Term Care Center focusing on rehabilitation therapies, skilled nursing, memory care, and more.
• Certifications Required: BLS
• Required Experience: Minimum of 1 year in Long-Term Care (LTC) or Skilled Nursing settings, specifically with Alzheimer's and Dementia populations.
• Must hold a valid New York State License; no license infractions allowed.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and report changes in resident conditions.
• Complete timely and accurate documentation.
• Provide nursing care in accordance with facility policies.
• Supervise Certified Nursing Assistants (CNAs) on assigned units.

Compliance

Compliance with facility policies regarding attendance and documentation will be strictly enforced. In the event of clinical issues, further investigation may be pursued according to the center's protocols.

Notes

First-time travelers are accepted. RTO (Request Time Off) is only approved at the time of submission and will not be accommodated once an offer is extended. Any call-outs must be provided with advance notice, over 24 hours prior to the scheduled shift.

Orientation

Orientation takes place every two weeks. Participation in orientation is mandatory and cannot be missed.

Work Environment

Facility features comprehensive services including long-term care, skilled nursing, memory care, wound care, and rehabilitation therapies.

Application Requirements

• Active New York State License required at submission.
• 1 verified reference required to submit, with a second required prior to the start of the contract.
• Candidates must complete necessary lab requirements within 48 hours post-acceptance to avoid cancellation.
