We offer supportive leadership, opportunities for professional growth, and the flexibility to maintain a healthy work-life balance.Team 10 covers Babylon, Bayshore, Brightwater, Deer Park, Gilgo Beach, Oak Beach, West Islip and the surrounding areas.If you're looking for a consistent weekend schedule with full-time benefits, this is the position for you!Schedule:Every Saturday & Sunday10.5-hour shifts7:00 AM – 5:30 PMEligible for full-time benefits as a 0.53 FTEJob Details:Case manages, assess, plan, develop and implement a comprehensive home health care plan for their patients.Measure vital signs, Administer IVs, change dressings, clean wounds as neededProvides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.Provide care to the most upmost standardMinimum Qualifications:New York State RN licenseMinimum of one year of recent hands-on experience as a Registered Nurse.Basic Life Support CertificationValid New York State Driver's License and reliable transportationSalary RangeUSD $100,000.00 - USD $130,000.00 /Yr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
