Case Manager — Benefits & Housing Advocate

You will help individuals and families apply for or recertify government benefits, conduct intake and assessment, and develop service plans to secure entitlements. As a team member you will manage a caseload, document progress in Salesforce, and prepare monthly reports while coordinating with other providers.
