Job Opportunity

Position: RN Home Health

This position involves coordinating and directing home nursing care based on physician's orders, following agency policies and procedures. The ideal candidate will demonstrate proficiency in the nursing process and be responsible for overseeing and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities

• Coordinate home nursing care delivery according to physician's orders.
• Supervise and evaluate Home Health Aides and Licensed Practical Nurses.
• Conduct home visits and complete necessary documentation promptly.
• Participate in periodic office meetings.

Travel Requirements

• Home visits are required, with a willingness to travel up to 75 miles from the assigned clinic.
• Must possess a reliable vehicle, current driver's license, and agency-required liability insurance.
• Mileage reimbursed at the IRS standard rate.

Patient Care and Case Types

• Average patient load: 5-10 patients per day.
• Types of cases include Start of Care, Wound Care, and Follow-ups.

Experience and Qualifications

• Minimum of 1 year of experience in Adult Home Health or Hospice.
• Must be proficient in HomeCare HomeBase EMR and Oasis.
• CPR and BLS certifications are required.
• Must have a valid nursing license in hand.

Work Schedule

• Standard schedule: Monday to Friday, 8:00 AM - 5:00 PM.
• Weekend rotation may be required, typically every fourth weekend.
• On-call duties may also be required, approximately every fourth week.

Location

The position covers the following territories: Hull, Halifax, Kingston, Plymouth, Pembroke, Marion, Mattapoisett, and Wareham, MA.

Additional Requirements

• All documentation must be completed within 24 hours of patient visits.
• Candidates must be comfortable with potential long drives between patient visits.
• Must understand that there are no guaranteed hours, with expectations to meet a productivity point requirement of 30 points weekly.
