Key Responsibilities\n

• Engage in regular meetings with multidisciplinary teams to assess and address client needs and concerns effectively.

• Craft and implement personalized treatment plans that target the unique needs and preferences of each client, continually monitoring progress and adjusting strategies as necessary.

• Educate clients and their families about available mental health resources and the emotional aspects of hospice care, ensuring they feel supported throughout the process.

• Maintain current knowledge of best practices and emerging trends in social work, particularly those relevant to hospice and palliative care settings.

• Document sessions and interventions thoroughly, adhering to organizational protocols related to patient privacy and record-keeping.

• Assist families with coordination of care and community resources, facilitating a smooth transition during critical times.

Patient Population / Client Focus\n

The primary focus of this role is on elderly populations facing terminal illness. As part of the hospice care team, the LCSW will work with patients and families grappling with challenging emotional and psychological issues, providing essential support that addresses grief, loss, and adjustment. Understanding the intricate dynamics associated with end-of-life, the LCSW plays a vital role in ensuring clients experience peace and dignity, along with emotional resilience during their transition.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is mandatory.

• An active Licensed Clinical Social Worker (LCSW) license in the state of Nevada is required.

Compensation and Benefits\n
