Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and families to identify strengths and challenges related to their emotional, social, and environmental contexts.

• Formulate, develop, and implement individualized intervention plans utilizing evidence-based social work practices.
\n
• Collaborate actively with healthcare providers, including physicians and specialists, to optimize patient care and communicate necessary information effectively.

• Facilitate discharge planning by coordinating with various stakeholders such as consulting physicians, insurance representatives, and community resources.

• Provide education and counseling to patients and their families about adjustment strategies for coping with their illnesses and life transitions.

• Utilize various therapeutic modalities, including crisis intervention and both individual and group therapies, to address patients' diverse needs.
\n
• Participate in an on-call rotation, providing after-hours support and intervention when necessary.
\n
• Evaluate oncology patients for potential mental health conditions such as depression and anxiety, offering short-term therapeutic support.
\n
• Facilitate involuntary examinations under the Baker Act when required and coordinate transfers when necessary.
\n
• Provide expert presentations and educational sessions on psychosocial oncology practices at community and professional gatherings.
\n
• Engage in community outreach efforts, contributing to educational, consultation, and research initiatives to strengthen local support programs for cancer survivors.

Patient Population / Client Focus\n

You will primarily work with a diverse clientele, including patients facing chronic illnesses such as cancer. Your role will be vital in addressing the psychosocial needs of these patients, providing them with the tools and understanding necessary to cope with their conditions, as well as their families. This focus on holistic care ensures that both emotional and practical needs are addressed throughout their treatment journey.

Qualifications\n

• A minimum of a Bachelor’s Degree in Social Work is required; a Master's degree is preferred.

• Current LCSW licensure in Texas is mandatory.
