Floor Nurse (RN/LPN) - Evenings Shift

Job Description

We are seeking dedicated Floor Nurses (RN or LPN) for an evening shift position in a Long-Term Care/SNF facility. In this role, you will provide direct patient care to residents, ensuring their comfort and safety. The ideal candidate will have prior experience with the Point Click Care (PCC) EMR system and a strong understanding of medication administration and patient assessment.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor vital signs and assist with activities of daily living (ADLs).
• Perform wound care, including measurement and documentation.
• Maintain accurate patient records and incident reports.
• Collaborate with interdisciplinary teams including physicians, therapists, and case managers.
• Participate in care plan meetings and provide input on patient progress.

Required Experience / Certifications / Licensure

• Must be a licensed RN or LPN with an active state license.
• BLS certification is required.
• Prior experience in a long-term care setting within the last three years is essential.
• Experience with PCC EMR and wound measurement preferred.

Schedule

This is a full-time position working 8-hour evening shifts from 2:45 PM to 11:00 PM, with every other weekend required.
