Description

• Conduct evaluations for patients in both inpatient and outpatient settings, tailoring individualized treatment plans to their needs
• Implement physical therapy interventions geared towards enhancing mobility, strength, balance, and overall function
• Collaborate with a diverse team, including nurses, physicians, case managers, and rehabilitation professionals, to improve patient outcomes
• Educate patients and their families on therapeutic objectives, discharge strategies, and exercises for home
• Document evaluations, treatments, and patient progress in alignment with facility protocols and regulations
• Engage in care team meetings and interdisciplinary discussions to promote coordinated healthcare

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., in neurology, geriatrics) are advantageous but not mandatory
