Work Environment

This onsite position allows you to work within our home care setting, providing flexible and personalized therapy services to patients. Key Responsibilities

• Conduct thorough evaluations by reviewing patient history and performing specific physical assessments to develop tailored treatment plans.
• Communicate evaluation findings and treatment strategies to patients and their families, involving them in goal-setting to ensure collaborative care.
• Implement treatment protocols using your clinical expertise to adapt to each patient's individual needs and progress, fostering positive outcomes.
• Consult with multidisciplinary healthcare professionals when necessary, documenting changes in the patient's condition and modifying care plans accordingly.
• Manage discharge planning and follow-up care, including timely documentation and encounter notes to maintain precise patient records.
• Exercise effective time management skills to meet productivity expectations while maintaining an organized schedule.
• Assist in overcoming barriers to patient access to care, engaging with marketing activities to promote our services in the community.
• Participate in departmental initiatives aimed at achieving specific goals, adhering to all regulatory and accreditation standards.
• Invest in your professional development by pursuing continuing education opportunities, and serve as a mentor for students in clinical training.
• Be proactive in promoting outstanding patient experiences through flexibility and responsiveness to their needs.

Patient Population / Client Focus

In this role, you will primarily work with a diverse population of patients requiring rehabilitation across various conditions, including orthopedic injuries, neurological disorders, post-operative care, and chronic pain management. Your ability to tailor therapies to meet individual patient needs will drive positive patient experiences and outcomes, allowing them to regain mobility and enhance their overall life satisfaction at home.

Qualifications

• A Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy is preferred.
• Current state license to practice Physical Therapy is mandatory.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

• Previous experience in outpatient or home care settings is advantageous, though new graduates are encouraged to apply.

Compensation and Benefits
