RN Care Manager - Inpatient

The RN Hospital Care Manager I delivers comprehensive care management services to patients in both inpatient and outpatient settings. Responsibilities include Conducting assessments, developing and adjusting patient-centered care plans, providing education and advocacy, and supporting self-management through motivational interviewing. They collaborate with multidisciplinary teams to ensure high-quality, efficient care, lead process improvement initiatives, and analyze data to meet regulatory standards. Qualifications A current RN license, a BSN (or obtaining one within four years), and clinical experience in a hospital setting with disease management knowledge.
