Description of Duties The Blended Case Manager (BCM) will promote resiliency and recovery for children, adolescents, their families and adults by accessing, coordinating and mentoring community resources and services. Qualifications A blended case management staff person shall meet one of the following criteria: A Bachelor’s degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education. Requirements Proof of education Act 33/34 clearances newer than 12 months FBI clearance Valid Pa drivers license Reliable transportation Proof of Pa insurance Physical health examination- within one year Effectively manage a case load of 25-30 Share the responsibility of 24/7 on-call coverage with the other case managers Attendance at weekly staffing and supervision meetings Participation in annual service trainings #J-18808-Ljbffr
