Community Health Advocate & Care Navigator

University of Maryland Medical System

The University of Maryland Medical System is seeking a Community Health Advocate to help patients adopt healthy behaviors and access community resources. This role requires acting as a patient advocate, scheduling appointments, conducting intake interviews, and providing referrals to community services. Ideal candidates will have a High School Diploma, experience in a transitional care setting, and strong communication skills. Knowledge of medical terminology is a plus.
