RN Coordinator - LV Durango Family Medicine Clinic - Full Time 8 Hour Rotating Shift (Exempt) ([...]

Responsibilities Serve as the RN Coordinator on the floor, providing guidance to RNs, LVNs, MAs, and other clinical or administrative staff. Address problems, issues, or concerns for physicians, staff, patients, and outside departments. Assist in management of clinical unit operations and oversee clinical floor staff and patient care coordinators across multiple practice sites. Act as the primary competency validator and educator for clinical staff. Perform telephone triage for same‑day appointments and oversee the triage nurse team. Provide therapeutic care using the nursing process in outpatient settings as required. Ensure patient privacy, safety, confidentiality, and compliance with infection control policies. Use evidence‑based, comprehensive care plans to manage patients and identify high‑risk, complex cases. Coordinate care, including referral management, care planning, post‑discharge planning, and transitional community services. Conduct individualized clinical evaluations, develop personalized action plans, provide education, and monitor progress. Serve as liaison between supervisors and subordinates, monitoring pending items and follow‑ups. Develop cohesive, team‑oriented relationships with providers and other healthcare professionals; evaluate patient health outcomes. Ensure exam rooms and treatment areas are supplied and maintained adequately. Lead discharge planning to support treatment adherence, medication compliance, and transitions for patients discharged from inpatient hospitals. Promote security by alerting personnel to safety, fire, and other unsafe situations; report unusual occurrences to supervisors. Respond to emergency situations using standard emergency protocols and procedures. Participate in monitoring and evaluating the quality and appropriateness of patient care; contribute objective data on clinical staff performance. Advocate on behalf of patients and families, coordinating community resources to ease transitions. Triage patient phone calls according to protocols and provide guidance and direction. Conduct nursing assessment, diagnosis, intervention, documentation, medication reconciliation, and patient education per care plans. Maintain compliance with national patient safety goals, including dual patient identifiers, procedural timeout, appropriate communication, medication reconciliation, and hazardous waste management. Participate in professional development, such as hospital or nursing activities and educational workshops. Qualifications Bachelor’s Degree in Nursing (BSN). Minimum of 2 years outpatient or inpatient clinical practice. Excellent communication skills in English, including speaking, reading, and writing. Knowledge of medical procedures and ability to perform whole‑blood collection and apheresis procedures. Preferred Qualifications Certified Case Manager (CCM, CMSA). Licenses & Certifications (Required) Registered Nurse (RN) licensed by the California Board of Registered Nursing. Basic Life Support (BLS). Registered Nurse, Nevada State Board. Fire Life Safety Training (Los Angeles City); if no card upon hire, one must be obtained within 30 days. Registered Nurse (CA) exempt from Nevada RN license.
